Service users guide

 

A referral may be made to request our service, privately or through direct payments.
The care co-ordinator or risk assessor will make an initail visit to your home to assess your needs, complete a risk assessment and discuss with you what care you would like, and together will complete a service user care plan.  Specific information on key policies and procedures will be discussed.

The Phoenix Care At Home Service provides a service of personal care, maintaining dignity at all times, allowing self respect to any person of any age with a disability, living in the community, whether living alone or within a family group.

We enable the person with disability to remain in their own homes for as long as possible, prevent or alleviate the stress of relatives and friends who have the continuing care of a person with a disability, by supplying a support worker to assist them with their care for an agreed time.

We aim to reduce social isolation and provide support for carers.  We provide flexible care packages of personal care, shopping, meal preparation, domestic duties, laundry, outings, or overnight care.

Our fees are attached and invoices are sent out every 4 weeks, in arrears.  Please pay within 7 days (by cheque made payable to Phoenix Care At Home Ltd) and posted to Unit B, cChivenor business park, Chivenor, Barnstaple. EX31 4AY.

Phoenix Care At Home has full employers liability insurance and also public liability insurance, with products liability and malpractice liability.

Our quality assurance process is achieved by phoenix care at home welcoming your views, and asking you for your comments annually, and by a visit to your home to complete a questionnaire.  Our staff all have regular supervision, training, personal development reviews, and attend staff meetings to ensure our services users have the best possible care.

We have a complaints procedure, a copy of which will be provided to you prior to our service commencing, which we encourage our service users to follow, should they have a complaint, as we are always striving to maintain best possible practice.  You may inspect the statement of purpose at any time, which is on display in the phoenix care at home office.
